Give to create a legacy.
The foundation has created a legacy society, called Circle of Caring, to recognize donors who have made provisions to support our hospitals in their estate plans. Circle of Caring donors leave their imprint on health care at Medical Center of the Rockies, Poudre Valley Hospital and Greeley Hospital through thoughtful, planned gifts from their estate. Their giving ensures that financial resources are available to provide health care that will meet the needs of future generations in our community.
Examples of such gifts include a will, trust, life insurance policy, retirement plan, or a life income plan, that benefits our local hospitals.
Members of the Circle of Caring receive special benefits, including:
Recognition at UCHealth Northern Colorado Foundation (or you can choose to remain anonymous).
A Circle of Caring lapel pin.
Invitations to special Circle of Caring events and other donor appreciation events and receptions.
If you have included Poudre Valley Hospital, Medical Center of the Rockies or Greeley Hospital in your estate plans and wish to inform us, please complete and submit the membership form. You can also call the foundation office at 970.237.7400 to ask for a form to be mailed to you. Your information is kept confidential. If you wish to remain anonymous, we will honor that decision.
Sample bequest wording: “I give, bequeath, and devise (dollar amount or description of asset) to UCHealth Northern Colorado Foundation, a qualified 501(c)(3) non-profit organization, located at 2315 E. Harmony Road, Suite 200, Fort Collins, CO 80528, to be used for its general purposes.”
Membership in the Circle of Caring is free and creates no obligation. If at any time in the future you decide to change your plans, simply notify the foundation office, and we will remove your name from the Circle of Caring membership list.
